The image of the entwined twins rests at the center. Above them the word, "MAY" and to the right, GEMINI are placed in relief. Below is the II glyph for the sign. Surrounding these are flowers and leaves.

Opening the locket reveals two compartments with frames and covers for a photographs or mementos. The reverse is highly polished and plain 14k gold.
